Patient Forms

If you are a new patient to our office, please click on the links below.  Please print them, fill them out and bring them with you to your appointment.  Please arrive at least 20 minutes early to allow proper time in transferring your important information.   

If you are a patient who recently received treatment with Dr. Thu, please review the Pre and Post Operative form for helpful information regarding planning for your visit and post operative care advice.   For patients scheduled for treatment, please review the Pre and Post Operative instructions and Consent forms for review in advance of upcoming procedures.  Depending on your procedure you will be asked to read, in advance, and sign a specific consent on the day of your visit.  You may print, sign and bring back to you on the day of your appointment or we can provide a copy to sign.
